Understanding Dental Insurance

Dental insurance plans provide coverage for individuals & families to help pay the costs of routine dental cleanings, x-rays, exams, basic and major dental procedures, and some plans also cover orthodontia.  Some insurance companies also provide an option to add vision and/or hearing benefits to that plan.

Understanding your dental insurance is the #1 way to get the maximum benefit from your dental coverage!

What do I need to know to choose the best dental plan for me and my family?

Here are some key points to understand and compare benefits across different plans:

Reimbursement levels & 3 Main Classes:

Dental insurance plans typically cover different groups of services at different coverage levels/percentages, and with different waiting periods. The services are split into 3 different “classes”. (Reference your carrier specific plan documents to see which services are covered in each class, as this can vary by plan/insurance company.)

  • Class 1:  Preventive Services (i.e. routine cleanings, x-rays, exams)
  • Class 2:  Basic Restorative Services (See plan outline for details and specific coverage/benefits.)
  • Class 3:  Major Restorative Services (See plan outline for details and specific coverage/benefits.)

Most plans offer preventive services (cleanings, x-rays and exams) on the first day of coverage, and some plans cover those costs at 100% with no deductible!   Read more below about waiting periods and deductibles.

Waiting Periods

In some cases, a plan will require a waiting period before it will provide coverage for certain treatments.  It is important to look at the plan’s waiting periods so that you know when you will be eligible to receive specific benefits.  Many plans do not have waiting periods for preventive services (routine cleanings, x-rays, exams).

Annual & Lifetime Maximums

The annual maximum is the maximum dollar amount that the insurance plan will reimburse annually, even if your dental costs exceed this limit.  Individual and Family dental plans typically offer a maximum annual benefit between $1,000 and $5,000.   

Some plans also have a lifetime maximum amount for specific covered services.  The most common dental service with a lifetime maximum is orthodontic treatment.

Deductibles

The amount of dental expense that you are responsible for before dental plan will start paying benefits.  This is typically between $0-$150 per individual, while some plans offer family deductibles as well.  It may be an annual or one-time charge (see your plan for details).

Coinsurance

Coinsurance refers to the amount (usually a percentage) that the patient is responsible to pay for a covered service, after the deductible has been paid.  

For example, if your plan specifies that it pays 80% for a filling, then your insurance plan would pay 80% (after your deductible has been paid), and you would be responsible to pay the remaining 20%.

In-network vs Out-of-network providers

Dental insurance plans typically have a network of providers (referred to as in-network providers) that have an agreed upon fees (plan’s allowed fee) for different procedures.  You will get the best rate and coverage by using an in-network dentist.

For example, if you see an in-network provider, and you need a filling, the plan may pay 80% of the provider’s contracted fee, and you are responsible for the remaining 20% of the provider’s contracted fee (after deductible).  However, if you see an out-of-network provider, the plan may pay a certain % based on UCR rates (usual, customary, and reasonable – see your outline of coverage), and you would be responsible for the difference up the provider’s full fee.  Seeing an out-of-network provider will result in higher fees.

Vision & Hearing Benefits

There are plans available that allow you to add an optional vision and/or hearing benefit to the base dental insurance plan.  These plans typically pay a set amount for a vision/hearing exam, as well as a maximum amount towards frames/lenses/contact lenses, and/or a maximum amount towards hearing aids.  Reference the plan brochures/outline of coverage for specific coverage amounts.
